The Bridge Program is an innovative, proven solution to provide high-quality primary care and
prevention services. We assist patients after a hospitalization, providing support and bridging the
gap from hospital to home.
We also provide in-home medical services, safety assessments, and referrals to health and
community resources. This allows patients to maintain optimal wellness in their own home. Save
healthcare dollars by preventing unnecessary 9-1-1 ambulance transports, emergency department
visits, and hospital readmissions.
The Bridge Program is open to any adult or child who resides in Lancaster County*, either in a private residence or a personal care facility. Get peace of mind knowing that you or your loved one can be cared for at home.
*Individuals with Medicaid insurance are not eligible for the Bridge Program. Contact us to discuss eligibility for other CP programs.
- Discharge instruction review and education after hospitalization

Total Cost: $500.00 per year
What's Included:
- Intake Visit (approximately 1 hour)
- 2 Additional Visits (approximately 30 minutes)
- Either follow up, skilled visits (blood draws, wound care, medication administration, etc), or a home safety assessment
- Discounted rates for any additional visits within the year
- $220 per 60-minute visit
- $135 per 30-minute visit
Founded in 1996, Lancaster EMS is a nonprofit organization providing primary 9-1-1
services to 21 municipalities encompassing over 275,000 Lancaster County residents. We
also offer non-emergency medical transports, an in-house EMT Academy, CPR classes,
and more.
Our Community Paramedicine (CP) Program was created based on the “safe landing”
methodology, which brings Community Paramedicine providers to the patient’s home for
follow-up care. We currently see more than 1,000 patients per month through the CP
program.
